Medical Treatments
At consultation, Dr. Tam will review the available medical options, as well as, hair restoration surgery. Medical treatments for hair loss can be used alone or in combination with surgery. Dr. Tam will often advise starting medical treatments to maximize the health of existing hair prior to surgery.
Rogaine
Rogaine (generic minoxidil) has been on the market for many years. It has a strong safety record because it is a topical solution and there is very minimal absorption into the bloodstream. There are two formulations, Rogaine 2% for women and Rogaine 5% for men. A foam formulation is also available for easier styling of hair. The foam formulation has less propylene glycol which decreases the risk of itchy, irritated scalp.
Rogaine may not help regrow hair that is lost, but it can help retain as much existing hair as possible. It helps to strengthen hair prior to surgery; it can be used alone or in combination for hair loss. Generally, we recommend starting Rogaine well before hair restoration surgery.
Propecia
Propecia (generic finasteride) is an oral medication that is used in men for hair loss and benign prostatic hypertrophy. It inhibits Type II 5 a-reductase which is the enzyme that converts testosterone into DHT. Since hair follicles are sensitive to DHT, Propecia may help retain existing hair. It works very well for the crown of the scalp and many men will need to continue use after surgery.
Spironolactone
Spironolactone is a diuretic that is occasionally used in women for hair loss, but is considered an off-label use. Spironolactone is an anti-androgen. Women who have an excessive production of male hormones may see a difference in hair diameter and growth rate with use of spironolactone. Laboratory and blood pressure monitoring is required while taking this medication. It is generally recommended that women also take an oral contraceptive or be postmenopausal while taking this medication.
Laser Treatments
Our office carries two laser products, the Hairmax Laser Comb and the Igrow system. Low level laser light has been shown to improve the diameter of the hair after 6-12 months of use. It is used three times per week for 15-20 minutes per treatment in the areas of hair loss. It is often recommended in conjunction with surgery and postoperatively. The laser comb is hand held but is smaller and more portable. The Igrow system is hands free but tends to be bulkier and not as easy for travel. Dr. Tam will discuss these options for patients who are candidates for hair laser.
